Un medico londinese specializzato nella cura delle paralisi viene invitato dalla sua ex fidanzata Maude, ora sposata con un barone, a recarsi nel loro castello che appare come un grande teschio in un paese mitteleuropeo. Quando arriva, il dott. Cargrave si accorge che nella dimora non ci sono specchi e che le cornici non hanno dipinti al loro interno. Non manca una stanza misteriosa che resta sempre chiusa.
